- inadequate arrangement of feed water inlets and connection of recirculation pipes;
- inappropriate design of the heated elements of the boiler;
- improper arrangement of gas ducts;
- defects of safety devices for furnaces and gas ducts;
- inconsistency of the burner devices with the design (type) of the boiler;
- inconsistency of the connection of boiler equipment with the wiring diagrams of the manufacturer;
- violation of the rules for placing boilers in the room;
- violation of the rules for the safe operation of boiler equipment.
